If you would like to move your healthcare from Friendship Square Medical to a different GP clinic, you can complete the form below and return it to us. We will then promptly arrange for a copy of your records to be sent to your new clinic.

You can complete this form if you would like to request a copy of the records Friendship Square Medical holds about you (for example if you are moving overseas or you would like a copy of records for your personal use).

If you are moving to another GP clinic within Australia, we suggest you instead complete a "Transfer of records from FSMC to another clinic" form, as we are often able to send your new clinic a copy of your records in their preferred machine-readable format.
